In case of medical reasons or old age, submit the following documents at the nearest Axis Bank branch to give a thumb impression if you are unbale to sign:


• Medical certificate stating the reason for the inability to sign should be original or a copy attested by the branch official

• Notarized signature change request form (Click Here to download)

• Proof Of Identity (The Aadhaar number should be masked if Aadhaar is chosen)

• The latest transaction statement of the demat account


The branch manager of the bank account specified in the demat account should attest the new signature.


Once we receive the documents the signature on the records will be updated in 3 working days.
