To demat shares held under your maiden name, you'll need to submit a notarized copy of your marriage certificate and a signed declaration to your Depository Participant (DP), along with the Dematerialization Request Form (DRF).
Submit the Dematerialization Request Form (DRF):
Click here for NSDL & Click here for CDSL to download a Dematerialization Request Form, this form is used to request the conversion of physical share certificates into a demat account.
Fill out the DRF:
Provide all the necessary details, including your maiden name (as it appears on the share certificates) and your current married name (as it appears on your demat account).
Attach Supporting Documents:
Notarized copy of your marriage certificate: This is crucial to prove the name change.
Gazette notification (if applicable): If you have officially changed your name in a gazette notification, provide a notarized copy of that as well.
Signed declaration: A declaration stating that you are requesting the dematerialization of shares held in your maiden name and that the name on the share certificates and the demat account are now the same.
Submit the Dematerialization Request Form (DRF):
Submit the DRF at the nearest Axis Bank branch, make sure you to obtain an In-Person Verification (IPV) and Original Seen Verification (OSV) stamp from the bank personnel, for your reference please do keep the acknowledgement shared by the branch personnel for your physical shares.
Please Note: Dematerialization requests take longer than usual as there is a dependency on the Registrar and Transfer Agency (RTA) of the share holding company, please give us a minmum of 7 days to process your request.
