You need to place an online Folio Consolidation request from your Axis Direct account and send the physical request. Please follow the below steps:

Step 1: Go to Trading => Mutual Funds => Folio Consolidation

Step 2: Place an Online Folio Consolidation request

Step 3: Go to Customer Service => Download Center => Other Forms & Downloads

Step 4: Download the Folio Consolidation application form => Fill the application form

Step 5: Send the application form, to the below mentioned address
